Job summary

Aspire Health and Community Services is seeking an experienced Licensed Practical Nurse (LPN) to join our Community Crisis Stabilization (CCS) team. In this role, you’ll provide specialized nursing care to adults in a 24-hour crisis stabilization program that serves as a medically necessary, voluntary, and less restrictive alternative to inpatient psychiatric hospitalization.

The CCS unit features a newly renovated 10 bed unit where individuals experiencing a behavioral health crisis receive

Qualifications

• Graduate of an accredited Nursing program (Diploma/Associates/or Bachelor’s degree).
• Licensed Practical Nurse (LPN) must be licensed.
• Ability to work independently, exercise sound clinical judgment, and provide compassionate, high-quality care.

Responsibilities

• Monitor and document clients' self administration of medications while ensuring safe medication practices.
• Monitor clients' physical health needs and coordinate acute medical care with direct care staff as needed.
• Facilitate health and wellness groups and activities that promote recovery and overall well-being.
• Deliver strength based, solution focused nursing interventions and support individuals in developing Wellness Recovery Action Plans (WRAP) and safety plans.
• Complete assigned clinical documentation and participate in required trainings to support high quality care.
