Job summary

Qualifications

• Graduate of an accredited practical nursing program.
• Active CT LPN license or eligibility.
• 1+ year clinical experience preferred.

Responsibilities

• Provides nursing care including medication administration and teaching.
• Contributes to care plan development and data collection.
• Communicates with residents, families and the health team.
• Documents information accurately in the EMR per regulations.
• Supervises CNA care to align with the care plan.
• Participates in mandatory inservices and quality initiatives.
