Job summary

Masonicare in Wallingford, CT is seeking a licensed practical nurse for an evening shift. In this role you provide nursing care under the supervision of an RN for assigned patients, deliver medications and treatments, and participate in developing the interdisciplinary care plan.

You will document care in the electronic medical record, supervise CNAs, and engage in ongoing education and quality improvement. CT LPN license and at least one year of hospital or facility experience preferred.

Qualifications

• Graduate of an accredited school of nursing for practical nursing.
• 1+ year of clinical experience in a hospital or similar health care setting preferred.
• Current State of CT license to practice as a Licensed Practical Nurse (LPN).

Responsibilities

• Provide nursing care under the supervision of an RN for assigned patients.
• Administer medications and treatments as ordered.
• Contribute to care plan development by collecting and recording data.
• Collaborate effectively with residents/patients, families, and interdisciplinary team.
• Supervise the care provided by CNAs to ensure compliance with the care plan.
• Document accurate information in the electronic medical record.
• Participate in education, inservices, and quality improvement programs.

Provides nursing care under the supervision of an RN, for assigned patients in assigned areas practicing Evidence-Based nursing as the standard.

Essential Duties And Responsibilities

• Provides nursing care appropriate to the age, needs and abilities of the resident/patient including:
• Medication & treatment administration
• Teaching
• Implementation of the interdisciplinary care plan.
• Contributes to the care plan development by collecting and recording data in an ongoing and systematic manner.
• Communicates and collaborates effectively with residents/patients, families, colleagues and other members of the interdisciplinary team to meet resident/patient and unit needs.
• Documents accurate, pertinent and comprehensive information in a timely and concise manner as required per regulation / facility P&P. Competently uses Electronic Medical Record.
• Supervises the care provided by the CNA's to assure compliance with the interdisciplinary care plan and resident profile.
• Participates in educational programs, inservices, meetings and workshops and utilizes current literature to enhance professional skills and knowledge. Attends all annual mandatory inservices.
• Participants in quality improvement programs as directed.

Minimum Qualifications

Education: Graduate of an accredited school of nursing for practical nursing
