Job summary

The Care Team Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to join our growing team for night shifts (10-12 hours), 3-4 nights per week, in New Philadelphia, OH.

You will provide hands-on nursing, monitor patients, manage symptoms, document care in EMR, and collaborate with an interdisciplinary team to deliver respectful, comfortable end-of-life care. LPN licensure and CPR certification required; hospice or home health experience preferred.

Qualifications

• Active LPN license required.
• Hospice, home health, long-term care, or skilled nursing experience preferred.

Responsibilities

• Provide skilled nursing care under RN direction following patient care plans.
• Monitor patient conditions and communicate updates to the care team.
• Assist with symptom management, medication administration, treatments, and patient comfort measures.
• Complete accurate documentation in the electronic medical record (EMR).
• Collaborate with physicians, RNs, caregivers, and the interdisciplinary team.
