Job summary

Oak Knoll Health and Rehabilitation is seeking a Licensed Practical Nurse (LPN) for the night shift (11PM–7AM) in a full-time capacity. You will provide direct nursing care, administer medications, and collaborate with RNs and the rehab team in a long-term care setting.

The role requires an active LPN license, BLS/CPR certification, strong clinical skills, and a compassionate, patient-focused approach. Competitive benefits are included.

Qualifications

• Current and valid LPN license.
• Clinical competencies including medication administration and wound care.
• Excellent communication and interpersonal skills.
• Team-oriented approach to care.
• BLS/CPR certification required.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor health status, vital signs, and document changes.
• Assist with ADLs such as bathing, dressing, and feeding.
• Collaborate with rehabilitation team to support recovery goals.
• Maintain accurate documentation per policies and regulations.
• Provide emotional support and education to residents and families.
• Ensure infection control and maintain safe environment.
