Job summary

Oak Knoll Health and Rehabilitation is hiring a Licensed Practical Nurse (LPN) for the Evening Shift (3PM–11PM). Full-time with competitive wages and full benefits. You will administer medications, monitor residents, and collaborate with RNs and therapists in a long-term care setting.

The role requires an active LPN license, BLS/CPR, and strong interpersonal skills to support residents and families in a compassionate, team-oriented environment.

Qualifications

• Current and valid LPN license required.
• Strong clinical skills including medication administration and wound care.
• Excellent communication and interpersonal skills; ability to work in a team.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health status and vital signs; report changes to supervising RN/physician.
• Assist with activities of daily living (ADLs) and support rehabilitation goals with the team.
• Maintain accurate documentation per facility policies and state/federal regulations.
• Provide emotional support and education to residents and families.
• Ensure infection control and a safe, clean environment.
