Qualifications

• Current, unrestricted LPN license in the state of employment.
• Graduate of an accredited practical nursing program.
• CPR certification current or willingness to obtain it.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Provide direct nursing care according to residents' care plans.
• Administer medications and treatments as ordered by physicians.
• Monitor residents' conditions and report changes to the clinical team.
• Document medications, treatments, observations, and responses.
• Assist with admissions, transfers, and discharges.
• Communicate with residents, families, and the interdisciplinary team.
• Supervise CNAs as assigned and ensure safety procedures.
• Follow infection-control and facility policies.
