Quality Center for Rehabilitation and Healing is hiring Licensed Practical Nurses (LPNs) for the 6:00 p.m.–6:00 a.m. night shift. The role focuses on resident-centered care in a rehabilitation and long-term care setting.

You will work with a multidisciplinary team to deliver medications, monitor health conditions, and document care plans. The ideal candidate holds a current LPN license in the state, has completed an accredited practical nursing program, and possesses strong clinical,

Qualifications

Current, unrestricted LPN license in the state of employment.
Graduate of an accredited practical nursing program.
CPR certification or willingness to obtain it.
Experience in skilled nursing, rehabilitation, or long-term care preferred.

Responsabilités

Provide direct nursing care in accordance with residents' care plans.
Administer medications and treatments as ordered by physicians.
Monitor residents' conditions and report changes to the clinical team.
Document medications, treatments, observations, and resident responses accurately.
Assist with admissions, transfers, and discharges as needed.
Communicate with residents, families, physicians, and the interdisciplinary team.
