Job summary

Bend Transitional Care in Bend, Oregon is seeking a dedicated Full-Time Registered Nurse (RN) to join our night shift team, 6:00 PM to 6:30 AM, three shifts per week.

Provide patient-centered care to residents, collaborate with the healthcare team, participate in care planning, medication administration, and discharge processes, all while upholding safety and quality standards. We offer a supportive work environment with union benefits and shift differentials.

Qualifications

• Requires nursing licensure in Oregon and completion of an accredited nursing program.
• Demonstrates compassionate, patient-centered care and strong communication.
• Ability to collaborate with the healthcare team and participate in care planning.

Responsibilities

• Provides direct care and/or assists with care as directed and within scope of practice.
• Initiates SBAR process and collaborates with RN for assessment and recommendations.
• Participates in resident evaluations and care planning; coordinates care and communicates changes to team/family.
• Administers medications as prescribed and documents in medical records; observes responses.
• Ensures timely documentation and maintains narcotic records; supports discharge planning.
