Job summary

Stonecreek in Paducah, KY is seeking a Night shift LPN/RN to deliver compassionate, resident-centered care in a 90-bed LTC setting. You will coordinate with the healthcare team, monitor vitals, administer medications, and support discharge planning while providing a safe, respectful environment.

Qualified candidates hold a current LPN or RN license in Kentucky, with 1 year LTC experience preferred; MatrixCare knowledge is a plus, and a supportive team culture awaits.

Qualifications

• Current LPN/RN license in Kentucky.
• At least one year of LPN or RN experience in a Long-term Care setting preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Consults and coordinates with healthcare team to assess, plan, implement, and evaluate patient care plans.
• Prepares and administers medications and records prescribed treatments; reports adverse reactions.
• Monitors patient vitals and records; manages equipment and IVs as needed.
• Participates in discharge planning and provides bedside care across medical patients.
• Performs duties such as dressing changes, catheter insertions, and assisting physicians during exams.

LPN/RN Core Responsibilities:

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans

• Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications. Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse.

• Records patients medical information and vital signs.

• Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays.

• Initiates corrective action whenever the patient displays adverse symptomatology

• Provides bedside care for a wide variety of medical patients.

• Changes dressings, inserts catheters, and starts IVs

• Prepares equipment and aids the physician during the examination and treatment of the patient

• Responds to life-saving situations based upon nursing standards and protocol

• Participates in discharge planning.

• Utilizes time management skills.

• Records all care information concisely, accurately and completely, in a timely manner, in the appropriate format, and on the appropriate forms.

• Performs other position-related duties as assigned, depending on assignment setting

LPN/RN Requirements:

• Current LPN/RN license in State.

• At least one year of LPN or RN experience in a Long-term Care Setting is preferred.

• Knowledge of MatrixCare preferred.
