Job summary

Stonecreek in Paducah, KY is seeking a Night shift LPN/RN to provide direct patient care in a 90-bed community. You’ll collaborate with the healthcare team to implement care plans, administer medications, monitor vital signs, and support discharge planning.

Qualifications

• Current LPN/RN license in state.
• At least one year of LPN or RN experience in a Long‑term Care Setting is preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans
• Prepares and administers prescribed medications and records, reporting adverse reactions
• Records patients' medical information and vital signs
• Monitors and adjusts equipment used on patients and interprets electronic displays
• Initiates corrective action when patient shows adverse symptoms
• Provides bedside care for a wide variety of medical patients
• Changes dressings, inserts catheters, and starts IVs
• Responds to life‑saving situations per nursing protocols
• Participates in discharge planning
• Records care information accurately and timely

LPN/RN Core Responsibilities:

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans

• Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications. Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse.

• Records patients medical information and vital signs.

• Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays.

• Initiates corrective action whenever the patient displays adverse symptomatology

• Provides bedside care for a wide variety of medical patients.

• Changes dressings, inserts catheters, and starts IVs

• Prepares equipment and aids the physician during the examination and treatment of the patient

• Responds to life‑saving situations based upon nursing standards and protocol

• Participates in discharge planning.

• Utilizes time management skills.

• Records all care information concisely, accurately and completely, in a timely manner, in the appropriate format, and on the appropriate forms.

• Performs other position‑related duties as assigned, depending on assignment setting

LPN/RN Requirements:

• Current LPN/RN license in State.

• At least one year of LPN or RN experience in a Long‑term Care Setting is preferred.

• Knowledge of MatrixCare preferred.
