Responsibilities

• Assess patient physical, psychological, social, and discharge planning needs.
• Formulate a discharge plan and care goals with patient/family input.
• Implement and supervise the plan of care in collaboration with the health care team.
• Evaluate patient progress and adapt the plan; provide patient teaching as needed.
• Document assessment, plan, and outcomes accurately in the patient record.
• Participate in unit leadership, quality improvement, and mentoring of staff.
