Job summary

Episcopal Homes of Minnesota is seeking a compassionate Registered Nurse for after-hours triage to support hospice patients and families during evening and overnight hours. As part of Beyond Hospice, you will perform comprehensive assessments, provide symptom management, and coordinate in-person visits when needed while operating with strong clinical judgment and a focus on comfort and quality of life.

Qualifications

• Graduate of an accredited nursing program.
• Current Minnesota RN license.
• Excellent assessment, observation, and documentation skills.
• Ability to work independently during evening and overnight hours.
• Valid driver’s license and reliable vehicle.
• Auto insurance that meets state requirements.

Responsibilities

• Perform comprehensive initial and ongoing assessments of patients and families.
• Assess concerns during after-hours triage calls and determine intervention level.
• Provide skilled nursing care focused on comfort and symptom management.
• Make in-person visits when clinically necessary during overnight shifts.
• Develop, implement, and update plans of care with the interdisciplinary team.
• Educate patients and families on medications and symptom management.
• Coordinate medications and supplies; ensure safe care.
