Job summary

Episcopal Homes of Minnesota is seeking an experienced Registered Nurse (RN) for after-hours triage in a hospice setting. This role focuses on telephone triage, symptom assessment, and when necessary, in-person visits to support patients and families during evening and overnight hours.

The ideal candidate holds a Minnesota RN license, can work independently, and has experience in hospice, home health, or triage.

Qualifications

• Current Minnesota RN license.
• Strong assessment and critical-thinking skills.
• Ability to work independently.
• Valid driver’s license and auto insurance.
• Hospice, home health, acute care, or triage experience preferred.

Responsibilities

• Provide after-hours telephone triage for hospice patients and families.
• Assess symptoms and determine appropriate interventions.
• Make in-person visits when needed.
• Provide comfort-focused nursing care and symptom management.
• Educate and support patients, families, and caregivers.
• Collaborate with physicians and the hospice interdisciplinary team.
• Complete timely and accurate clinical documentation.
