Job summary

Three Oaks Hospice, Inc. is seeking an On Call Registered Nurse (RN) to join our team delivering compassionate hospice care in patients' homes across the designated counties.

You will administer skilled nursing services, coordinate with the interdisciplinary care team, and support patients and families while prioritizing comfort and dignity. This role requires current RN licensure, CPR, and at least two years of clinical RN experience.

Qualifications

• Graduate of an accredited Associate or bachelor's degree in Nursing.
• Current state RN license and driver's license.
• Two years RN experience in clinical care; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office preferred.

Responsibilities

• Administer skilled nursing care for clients in their residence.
• Coordinate care with the interdisciplinary team and family; manage care coordination.
• Support patients and families with clinical needs and education.
