Qualifications

• Provides hospice/palliative care using the nursing process
• Assesses patient and family needs; develops and updates Plan of Care
• Coordinates with interdisciplinary team and coordinates care with physicians
• Educates patient and family; supports bereavement follow-up and resources
• Maintains thorough documentation and complies with regulatory requirements
