Job summary

Prisma Health Hospice is seeking a registered nurse to provide compassionate hospice and palliative care to patients and supportive care to families in the Upstate region. The role follows the nursing process, adheres to policies, and requires timely documentation and coordination with the medical team.

Minimum of Associate Degree in Nursing with 1 year of nursing experience; BSN preferred. Current SC RN license and valid driver’s license required.

Qualifications

• Education: Associate degree in Nursing; 1 year nursing experience. Hospice or home care preferred.
• BSN preferred.
• Must hold a current RN license recognized in SC.

Responsibilities

• Assess patient physiological, psychosocial, developmental, and family needs to inform the Plan of Care.
• Develop and update a nursing plan of care and CNA plan as needed.
• Document assessments clearly in the medical record and communicate with physicians.
• Provide education to patient/family and coordinate interdisciplinary care.
• Utilize standing orders and guidelines to guide practice and ensure quality care.
