Job summary

Prisma Health is seeking an experienced Hospice RN to provide comprehensive end-of-life care in the Hospice House, coordinating with the interdisciplinary team to meet patient and family needs. You will document assessments, manage care plans, and educate families while adhering to organization policies.

This role requires an Associate degree in Nursing, an active RN license, and capacity to work in a fast-paced hospice setting. Two years of acute care nursing is preferred, with BSN encouraged.

Qualifications

• Associate degree in Nursing required.
• Two years Acute Care Nursing experience preferred; hospice/home care experience preferred.
• RN license active or multi-state recognized where practicing.

Responsibilities

• Assess patient physiological, psychosocial, developmental, environmental needs and family support requirements; contribute to interdisciplinary Plan of Care.
• Develop and update individualized nursing and CNA care plans with input from patient and family.
• Educate patient and family; refer to other hospice team members as needed.
