Job summary

SSM Health's WI-Hospice Home of Hope in Wisconsin is hiring a Regular Nurse Aide for a full-time night shift. You will join a compassionate hospice care team to support patients and families with dignity and respect.

In this role you provide essential nursing care under supervision, assist with daily tasks, monitor conditions, and help with discharge preparation, while adhering to safety and quality standards.

Qualifications

• Completion of approved nurse aide training program and passing grade on state exam.
• No prior experience required.
• BLS HCP certification is required in addition to CNA.

Responsibilities

• Receives patient care assignment from the RN in charge and initiates delegated tasks.
• Documents data for admissions and reassessments for analysis and validation of patient response.
• Contributes to safety and quality care by relaying information via charting and communication to team members.
• Informs licensed nurse of changes in patient condition or needs and patient education requirements.
• Assists with activities of daily living and routine care including bathing, feeding, hygiene, and mobility.
• Prepares patients for discharge and accompanies them upon dismissal as indicated.
