Job summary

Transitions Hospice is seeking a compassionate Companion Aide CNA for the night shift (8 p.m. to 8 a.m.). You will provide bedside presence, monitor patient safety, assist with personal care, and document observations while supporting families overnight.

Required CNAs must hold current state certification and CPR. Prior hospice or home health experience is preferred, with the ability to work independently during overnight hours.

Qualifications

• Current CNA certification meeting state requirements.
• CPR certification required or must be current.
• Experience in hospice, home health, or similar setting preferred.
• Willingness to work overnights and stay alert.

Responsibilities

• Provide continuous bedside presence and patient supervision overnight.
• Monitor patient condition, behavior, and environment and report changes to on-call nurse.
• Assist with personal care including repositioning, toileting, bathing, hygiene, and dressing.
• Record vital signs as ordered and maintain accurate documentation.
• Support families with overnight respite and compassionate communication.
