Responsibilities

• Receives patient care assignment from the registered nurse (RN) and initiates delegated tasks based on demonstrated competence in nursing activities for the patient age group served.
• Collects and documents data for the admission assessment and reassessment at specified times for the patient population; provides data to the RN/LPN for analysis and validation of the patient’s response to care.
• Contributes to safety and quality of care by relaying information via charting and communication to team members.
• Informs licensed nurse of changes in patient condition/needs, patient complaints, and educational needs of patient/significant others.
• Assists with baths, mobility, feeding, skin care, hygiene and bed-making; performs dressing changes, enemas, ostomy care, toileting and catheter care.
• Prepares patients for discharge and accompanies them upon dismissal as indicated.
• Stocks and maintains supplies and equipment; uses time and resources efficiently.
• Performs delegated clerical duties such as answering phones, relaying messages, transcription of physician orders, and organizing patient records.
