Qualifications

• Graduation from a state accredited school of nursing.
• Successful completion of orientation and probationary period.
• Annual continuing education including Fire Safety and BLS/CPR recertification.

Responsibilities

• Performs initial assessment and ongoing monitoring for complex patients.
• Provides clinical and psycho-social services per the plan of care.
• Records observations and treatments in PointCare EMR.
• Coordinates with interdisciplinary team, Medical Director, and attending Physician.
• Educates patient and family on care, disease process, symptom control, and wound care.
• Completes and submits required documentation by deadlines.
