Job summary

Good Shepherd Health Care in Hermiston, Oregon seeks a Home Health and Hospice Registered Nurse to assess, delegate, coordinate, and evaluate nursing care in patients' homes. The RN initiates the plan of care, ensures quality care, and coordinates services from multiple disciplines, reporting to the Home Health and Hospice Manager.

This night-shift position runs 4:30 pm–4:00 am with a 7 nights on/7 nights off schedule, and on-call duties.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license in Oregon; CPR certification; valid Oregon driver’s license and proof of auto insurance.
• One year of RN experience in an acute care setting; home health experience preferred.

Responsibilities

• Provide essential medical care to patients in their home setting.
• Assess patient’s physical needs and develop an individualized care plan.
• Clear communication and collaboration with patient, family, caregivers and providers.
• Communicate observations and treatment suggestions within the interdisciplinary team.
• Educate patients and families on disease and symptom management.
• Maintain timely, detailed records of patient assessments, care plans, and outcomes.
• Advocate and provide support for the patient.
• Support hospital mission, vision, values, policies, and procedures.
• Participate in required education for relevant programs.
• Perform other related duties as assigned.
