Job summary

Good Shepherd Health Care System is seeking an experienced Home Health Registered Nurse to deliver in-home clinical care and coordinate plans of care for patients in their homes and community settings. The RN will assess needs, create individualized care plans, and collaborate with families and the healthcare team to ensure quality outcomes.

Qualifications include an active Oregon RN license, CPR, and at least 1 year of acute care experience; BSN is preferred.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license in Oregon; CPR certification.
• One year RN experience in an acute care setting.
• BSN is preferred.

Responsibilities

• Provide essential medical care and support to patients in their home setting.
• Conduct ongoing assessments and develop individualized care plans.
• Collaborate with patient, family, caregivers, and providers.
• Communicate observations and treatment suggestions to the interdisciplinary team.
• Provide education to patients and families on disease and symptom management.
• Maintain accurate patient records and care plans.
