Job summary

Qualifications

• Graduate of an accredited school of professional nursing.
• BSN preferred.
• Current RN license in Oregon; CPR certification; valid Oregon driver's license and proof of auto insurance.

Responsibilities

• Provide essential medical care and support to patients in their home setting.
• Conduct ongoing assessments and develop individualized care plans.
• Communicate observations and treatment plans with the interdisciplinary team.
• Educate patients and families on disease and symptom management.
• Maintain accurate records of assessments, care plans and outcomes.
• Advocate for and support the patient.
