Job summary

Trinity Health Grand Rapids is seeking a Part-time Social Worker for the Emergency Department/Inpatient unit. The role requires a Master’s in Social Work (MSW) and Michigan licensure, with 2–5 years of hospital social work experience preferred.

The position involves psychosocial assessments, discharge planning, and coordination with the care team to support patients and families. The successful candidate will navigate complex psychosocial needs, work across disciplines, and advocate for patients

Qualifications

• MSW is required.
• Current Limited License in Social Work in Michigan is required.
• Two to five years of Social Work experience in a health care setting.
• Hospital social work experience is preferred.

Responsibilities

• Utilize Social Work Expertise, Psychosocial Assessment Skills, and Critical Thinking.
• Conduct comprehensive psychosocial assessments to identify patient, family, social, behavioral, financial, environmental, and community resource needs.
• Identify barriers to care, complex psychosocial concerns, safety risks, and social determinants of health.
• Assess patient and family needs and develop discharge and transition plans using evidence-based practice.
• Coordinate and collaborate with patients, families, multidisciplinary team members, and community agencies.
• Serve as a patient and family advocate and educate on available resources and transition-of-care options.
