Job summary

Trinity Health Grand Rapids Hospital is seeking a Social Worker with a Master’s in Social Work (MSW) to support the ED and inpatient units. The role involves psychosocial assessments, discharge planning, and advocacy to help patients and families navigate care and resources.

You will work 3 twelve-hour shifts weekly with various mid or night hours, including Sundays, contributing to high-quality, patient-centered care in a fast-paced environment.

Qualifications

• Master's Degree in Social Work (MSW) required.
• Current Limited License in Social Work in the State of Michigan.
• 2–5 years of social work experience in a health care setting preferred (hospital).
• Ability to work in a fast-paced ED/inpatient environment and manage complex cases.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify patient and family needs affecting care, discharge planning and resources.
• Identify barriers to care, safety risks, and social determinants impacting outcomes.
• Develop individualized discharge and transition plans using evidence-based practice.
• Manage caseload according to organizational standards and regulatory requirements.
• Intervene in crisis situations and complex psychosocial challenges; provide patient and family advocacy.
