Job summary

Trinity Health Grand Rapids Hospital is seeking a Social Worker with an MSW to join the ED/inpatient team on a part-time, 12-hour night shift. The role focuses on psychosocial assessment, discharge planning, and care coordination for patients and families in a fast-paced environment.

You will conduct comprehensive assessments, identify barriers to care, and develop individualized plans while collaborating with physicians, nurses, and community services to support optimal outcomes.

Qualifications

• MSW required; preferred: Michigan licensure or ability to obtain
• Two to five years of Social Work experience in healthcare settings
• Experience in ED/inpatient environments is a plus

Responsibilities

• Conduct comprehensive psychosocial assessments identifying patient/family needs
• Develop individualized discharge and transition plans
• Manage cases per standards and regulatory requirements
• Intervene in crisis, safety, and complex psychosocial situations
• Coordinate with multidisciplinary team and community resources
• Advocate for patients and families while promoting informed decision-making
