Job summary

Trinity Health Grand Rapids Hospital seeks a part-time Social Worker with an MSW to join the ED/inpatient team. You will perform psychosocial assessments, discharge planning, and crisis intervention, coordinating with physicians, nurses, and community resources.

Qualifications

• Master’s degree in Social Work required; Michigan licensure preferred.
• Two to five years of social work experience in a health care setting preferred, hospital background beneficial.

Responsibilities

• Conduct psychosocial assessments to identify patient, family, social, economic, and environmental needs impacting care.
• Identify barriers to care and safety risks affecting outcomes.
• Develop individualized discharge and transition plans based on evidence-based practice.
• Manage cases per organizational standards and regulatory requirements.
• Intervene in crises, abuse/neglect, homelessness, substance use, and other psychosocial challenges.
• Provide active listening and supportive interventions to assist coping with illness and hospitalization.
• Coordinate with patients, families, and multidisciplinary teams across care settings.
• Advocate for patients/families and promote autonomy and informed decision-making.

Job Title: Social Worker Masters

Work Location: Trinity Health Grand Rapids Hospital

Department: ED/inpatient

Shift: 2 12-hour shifts/week 7p-7a

What You Will Do

• Conduct comprehensive psychosocial assessments to identify patient, family, social, behavioral, financial, environmental, and community resource needs that may impact care, treatment, and discharge planning.
• Identify barriers to care, complex psychosocial concerns, safety risks, and social determinants of health that may affect patient outcomes.
• Assess patient and family needs and develop individualized discharge and transition plans by utilizing evidence-based practice, professional knowledge, and skilled intervention.
• Manage assigned cases according to organizational standards, regulatory requirements, and professional social work practice standards.
• Intervene in complex situations involving crisis management, adjustment to illness, family dynamics, abuse or neglect concerns, behavioral health needs, guardianship issues, end-of-life planning, homelessness, substance use disorders, and other psychosocial challenges.
• Engage in active listening and supportive interventions to assist patients and families in coping with illness, hospitalization, and life-changing events.
• Coordinate and collaborate with patients, families, multidisciplinary team members, post-acute providers, community agencies, and other stakeholders
• Build strong professional relationships, advocate effectively, negotiate solutions, and work collaboratively with individuals from diverse cultural, socioeconomic, and educational backgrounds.
• Establish and promote collaborative relationships with physicians, nurses, case managers, therapists, community agencies, and other members of the healthcare team.
• Serve as a patient and family advocate.
• Educate patients and families regarding available resources, healthcare decision-making, advance care planning, community services, and transition-of-care options.
• Promote patient autonomy, self-determination, dignity, and informed decision-making while supporting compliance with applicable laws, regulations, and organizational policies.

• Perform other related duties as assigned.
