Job summary

Trinity Health Grand Rapids is seeking a Social Worker with a Master’s in Social Work to join our inpatient team for weekend coverage.

This part-time role provides psychosocial assessments, brief counseling, and care transitions planning for patients and families, with Saturdays and Sundays 8:00 AM–4:30 PM shifts.

Licensed social work eligibility in Michigan is required, with at least 2 years of healthcare experience preferred; strong communication and teamwork are essential.

Qualifications

• Master’s Degree in Social Work from an accredited institution.
• Eligible for LLMSW or LMSW licensure by the Michigan Board.
• Minimum of 2 years of social work experience in a healthcare setting; emergency/behavioral health experience preferred.

Responsibilities

• Provide social work services during weekend shifts (8:00 AM – 4:30 PM) in the inpatient hospital.
• Conduct psychosocial assessments for patients and families in crisis.
• Develop individualized treatment and discharge plans addressing emotional, social, and practical needs.
• Deliver brief counseling and crisis intervention.
• Coordinate with community agencies and internal resources to support safe care transitions.
• Serve as patient advocate and liaison between care teams and families.
• Document assessments, plans, interventions, and outcomes in a timely manner.
• Collaborate with the interdisciplinary care team to support holistic patient care.
