Job summary

Haven Place is seeking a compassionate Certified Nursing Assistant (CNA) to join our care team on the 11p-7a shift. You will deliver direct patient care, assist with ADLs, monitor vital signs, and support our residents' wellbeing.

You will collaborate with the interdisciplinary team to develop care plans, document care activities, participate in medication management under supervision, and help maintain a positive, safe environment for patients and families.

Qualifications

• Certified Nursing Assistant (CNA) certification
• Ability to work 11pm-7am shift
• Strong communication and documentation skills

Responsibilities

• Provide direct patient care, including assisting with ADLs, vital signs, and other basic care needs
• Develop and implement personalized care plans in collaboration with the interdisciplinary team
• Maintain accurate and detailed records of patient care and treatment
• Participate in medication management, including administration and monitoring of medications
• Foster a positive and supportive environment for patients, families, and staff
• Collaborate with other healthcare professionals to ensure seamless care delivery
