Job summary

Legacy Living and Rehabilitation Center in Wyoming seeks a dedicated CNA to provide basic resident care, assist with ADLs, and ensure comfort and safety for residents. You will work under a licensed nurse, document clearly, and support the care team according to CMS guidelines.

Responsibilities include assisting with admissions/discharges, aiding nutrition staff, and maintaining a clean environment. CPR/AC certifications are expected as per policy; previous CNA experience is preferred.

Qualifications

• Must have a current Wyoming CNA certificate with on-time renewal.
• Graduate from a state-approved CNA training program with competency evaluation.
• Wyoming CNA certification and registration in the Wyoming CNA abuse registry.
• Previous CNA experience preferred; AC/CPR certifications as required.

Responsibilities

• Assist with basic resident care and ADLs per care plan.
• Observe residents and report unusual conditions to the Charge Nurse.
• Attend bedside shift huddles and participate in rounds for safety.
• Assist with admissions, discharges, and transfers.
• Assist nutrition staff with meal trays and setup per plan.
• Document ADLs, behavior, baths, skin integrity, and other records.
• Maintain a clean and safe environment and manage supplies.
