Job summary

You will support residents with ADLs, monitor condition changes, and ensure safety and comfort under the guidance of the Charge Nurse.

The role emphasizes maintaining the Plan of Care, effective communication with the nursing team, and providing compassionate care to residents, including end-of-life support when needed.

Qualifications

• High school diploma or equivalent required.
• Certified Nurse Assistant (CNA) in good standing on the Ohio Department of Health Nurse Aide Registry required.
• Experience: None required; on-the-job training provided.
• Must be able to read, write, speak, and understand the English language.

Responsibilities

• Care Management and Service Delivery duties including daily resident and patient care as instructed by Charge Nurse.
• Maintain Plan of Care for each resident or patient.
• Provide Activities of Daily Living (ADL) support including hygiene, mobility, nutrition, and safety.
• Assist with care of dying residents and post-mortem care.
• Observe and report pressure areas or skin breakdown.
• Assist with nursing treatments and restorative procedures.
• Create a warm, caring atmosphere and respond to resident calls promptly.
