Qualifications

Responsibilities

• Provide daily resident and patient care per nursing procedures and Charge Nurse instructions.
• Maintain Plan of Care for residents/patients.
• Assist with Activities of Daily Living (ADL) including hygiene, elimination, nutrition and hydration.
• Assist with dying patient care and post-mortem care as needed.
• Observe skin integrity and report pressure areas.
• Support with nursing treatments and restorative procedures.
• Record ADL, intake/output, temperatures and weight in EMR.
• Report condition changes to the Charge Nurse; participate in care conferences and meetings.
