Qualifications

• PharmD degree and active pharmacist license.
• Experience in hospital pharmacy preferred.
• Strong communication and teamwork skills.

Responsibilities

• Provide comprehensive pharmaceutical care to patients and healthcare team.
• Evaluate and verify medication orders for safety and efficacy.
• Supervise technicians and provide clinical interventions as needed.
• Formulary management and cost-effective pharmacotherapy.
• Document interventions in EMR and CDS systems.
• Support nursing and physicians with medication-related inquiries.
• Contribute to neonatal, pediatric, adolescent, adult and geriatric pharmacotherapy.
• Participate in drug information, emergencies, and drug safety activities.
