Job summary

Trinity Health Ann Arbor seeks a Clinical Pharmacist for the Midnight shift to provide medication preparation and distribution, and to participate in pharmaceutical care for diverse patient populations in an acute care setting.

The role includes rounds, TDM, patient counseling, antimicrobial stewardship, and collaboration with the healthcare team to ensure safe, effective, and cost-efficient pharmacotherapy. Requires Pharm.D or clinical Master’s and Michigan licensure.

Qualifications

• Requires Doctor of Pharmacy Degree (Pharm.D) or clinical oriented Master's Degree in hospital pharmacy from an accredited College of Pharmacy or equivalent.
• Minimum of two years related experience, which may include a clinically oriented hospital pharmacy residency (PGY1).
• Current Michigan Pharmacist license and continuing education credits as required by the State; BLS/ACLS certification as required by hospital policy.

Responsibilities

• Interpret and evaluate medication orders for appropriateness of therapy.
• Monitor patient medication regimens for interactions, contraindications, and adverse drug reactions.
• Individualize therapy based on patient variables (renal function, age, weight, labs).
• Manage drug therapy across the continuum of care and intervene on unclear or inappropriate orders.
• Participate in rounds, therapeutic drug monitoring, patient education, antimicrobial stewardship, and drug information education.
