Qualifications

Responsibilities

• Leads coordination of nursing care with other disciplines and facilitates integration across the continuum of care.
• Collaborates with staff and departments to facilitate appropriate and timely patient placement.
• Promotes a culture of safety and ensures compliance with initiatives and quality metrics.
• Maintains patient rights, privacy, and confidentiality while supporting staff and leadership.
• Monitors staffing and resources to optimize quality, cost-effective care and credentialing across the unit.
