Qualifications

• Master's Degree in a mental health field (Social Work, Counseling, Psychology or related) is required.
• 0–3 years of relevant experience.
• License eligible in Indiana as LCSW/LMHC/LMFT/LCAC; must have a permanent LSW/LMHC-A/LMFT-A upon hire.
• Certification in crisis intervention may be required.
• BLS certification may be required.
• Knowledge of psychosocial assessment, therapeutic counseling and crisis intervention in health care settings.
• Knowledge of Evidenced Based Practices (CBT, MI, DBT).
• Knowledge of state/county adult and child protective legal mandates.
• Knowledge of growth and development across the life span.
• Ability to maintain crisis intervention and de-escalation techniques.
• Ability to provide a safe, therapeutic environment; instruction of daily activities and programming.
• Basic proficiency in Electronic Medical Record and MS Office.
• Ability to develop and maintain strong working relationships with community, medical teams, law enforcement, APS/CPS, schools, probation and other agencies.

Responsibilities

• Facilitates individual, group, and family counseling.
• Performs utilization review and discharge planning.
• Collaborates with the health care team to ensure safe, timely care.
• Interprets and communicates complex patient/family needs to caregivers.
• Assesses psychosocial risk factors and mobilizes resources to meet identified needs.
• Provides mental health assessments upon physician referral.
• Intervenes to facilitate complex discharge planning.
• Serves as liaison with protective service teams and responsible for related cases.
