Job summary

Qualifications

• Master's degree in a mental health field such as Social Work, Counseling, Psychology or related field is required.
• Requires 3-5 years of relevant experience.
• Requires current license in Indiana as LCSW, LMHC, LMFT, LCAC or HSPP.
• Crisis intervention certification may be required.
• BLS certification may be required.
• Experience in health care, mental health, or community-based settings.
• Knowledge of evidenced-based practices (CBT, MI, DBT).
• Knowledge of state/county protective mandates for adults/children.

Responsibilities

• Facilitates individual, group, and family counseling.
• Performs utilization review and discharge planning.
• Collaborates with health care team to ensure safe, timely, and comprehensive care.
• Interprets and communicates complex patient and family needs.
• Acts as liaison with protective services for abuse cases and guardianship/adoption placements.
• Accesses and mobilizes family/community resources to meet needs.
• Provides mental health assessments on physician referral.
• Provides intervention to support complex discharge planning.
