Qualifications

Responsibilities

• Assess patient physical, psychological, social and discharge planning needs.
• Formulate/implement plan of care and discharge planning with the care team.
• Provide direct patient care and coordinate activities of health care team.
• Document assessment, care provided and patient response.
• Promote continuity of care and effective patient education.
