Qualifications

Must possess a current, valid RN license in state of practice.
BLS certification is required.
Relates throughout the interview process the experience, training and education needed to perform the job.

Responsibilities

Assess patients' physical, psychological, social, spiritual, educational, developmental, cultural, and discharge needs.
Formulate and update the care and discharge plan with input from patient, family and health team.
Implement care, coordinate care, and supervise the health care team as required.
Evaluate patient progress and adjust care and teaching as necessary.
Document assessment, planning, implementation and evaluation in the patient record.
