Responsibilities

• Performs ongoing and initial health status assessments and documents data from patients and families.
• Documents planned nursing interventions and coordinates care plans and medical management.
• Evaluates response to interventions and revises plans as needed in collaboration with the health care team.
• Delegates tasks according to state statutes and coordinates continuity of care.
• Participates in process improvement, safety and efficiency to achieve quality and financial targets.
