Job summary

Transitions Hospice in Springfield, IL is seeking a Registered Nurse for the after-hours night shift. As a Night Nurse, you will provide overnight nursing assessments, treatments, and compassionate care within an interdisciplinary team to support patients and families in Sangamon and surrounding areas.

You will administer meds, document care, communicate changes to physicians, and coordinate with community partners to ensure continuity of care across Sangamon and surrounding areas.

Qualifications

• Active RN license in the state of employment
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or related setting preferred
• Comfort and composure working independently during overnight hours
• Strong assessment skills and sound clinical judgment in high-pressure moments
• A calm, reassuring presence for patients and families during nighttime distress
• Excellent documentation habits and communication skills
• Valid driver's license and willingness to travel locally

Responsibilities

• Deliver nursing care across the full nursing process within scope of practice
• Conduct patient and family assessments and contribute to individualized care plans
• Complete all visit types as needed to meet overnight patient and family needs
• Administer medications and treatments per physician orders
• Identify nursing diagnoses from health assessment data and respond with sound clinical judgment
• Record accurate, timely clinical notes including pain and symptom management outcomes
• Communicate changes in patient condition to physicians and obtaining orders as required
• Coordinate with community health partners to support continuity of care
• Ensure equipment and necessary services are in place for patients
• Maintain and grow your clinical skills through continuing education and in-service participation

Education

Graduate of an accredited school of Nursing.
