Job summary

Transitions Hospice in Bloomington, IL is seeking a Registered Nurse for after-hours coverage. You will provide overnight nursing care from 8 PM to 8 AM, with a rotating schedule of 4 days on and 3 days off, including weekends.

Coverage areas include Bloomington and nearby communities; you will work with an interdisciplinary team to assess, treat, and support patients and families through the night. Applicants should hold an active RN license in Illinois and have hospice or related clinical

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.

Responsibilities

• Deliver professional nursing care across the full nursing process within your scope of practice.
• Conduct patient and family assessments and contribute to individualized care plans.
• Complete all visit types to meet overnight patient and family needs.
• Administer medications and treatments per physician orders.
• Record accurate clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners to support continuity of care.
• Ensure equipment and services are in place for patients.
• Maintain and grow your clinical skills through continuing education.
