Job summary

You will monitor symptoms, coordinate resources, and participate in team meetings to implement individualized treatment plans while maintaining confidential documentation.

Qualifications

• High school diploma or equivalent required.
• Three years of relevant behavioral health experience recommended.
• Ability to document accurately and maintain records.

Responsibilities

• Provide case management services to children and families via telehealth, offices, or community settings.
• Monitor client symptoms and communicate with treatment team for interventions.
• Coordinate resources with schools and outpatient providers to ensure continuity of care.
