• Develop and implement a care plan with discharge planning and regular plan evaluation.
• Conduct ongoing infant assessments and communicate changes to providers.
• Administer medications and treatments per hospital policy.
• Support transfers by stabilizing infants and documenting handoffs.
• Provide family education on homecare, treatments and community resources.
