Advocate Health is seeking a licensed Physical Therapist to interpret physician referrals and perform comprehensive evaluations across musculoskeletal, neuromuscular, cardiopulmonary, and integumentary systems. You will establish and implement a personalized treatment plan, monitor outcomes, and adjust care as needed to meet functional goals.

Qualificações

• Doctorate in Physical Therapy required.
• Licensed Physical Therapist in the state of practice required.
• BLS for Healthcare Providers certification required or obtainable within 90 days.

Responsabilidades

• Interpret referrals and perform musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations.
• Establish a plan of care and implement individualized treatment based on evaluation results and goals; monitor outcomes.
• Plan discharge including home programs, patient/family teaching, and referrals; coordinate with care team.
