Job summary

You will document progress, adjust plans as needed, and participate in program meetings to drive continuous quality improvements. A Louisiana licensure and BLS are required, with a commitment to excellent patient care.

Qualifications

• Demonstrates skill and accuracy in performing age-appropriate assessments and documents evaluation in EMR.
• Reassesses and revises goals and treatment plan as patient status and CMS rules require.
• Collaborates with physicians and staff to coordinate care and ensure quality outcomes.

Responsibilities

• Evaluate patient progress and adjust treatment plans accordingly.
• Plan and implement age-appropriate therapies and discharge plans.
• Provide guidance to staff and participate in team conferences to improve care quality.
