Qualifications

• RN Home Care assessment and evaluation in home setting.
• Nursing education delivery to patients/families.
• Care planning with priorities and resource management.
• Evidence-based nutrition practices in patient care.

Responsibilities

• Collaborates in establishing the plan of care and coordinates care delivery.
• Analyzes patient information to determine nursing diagnoses.
• Evaluates patient progress and adjusts interventions to achieve outcomes.
• Promotes health and a safe environment for patients and families.
