Care Ring, Inc. in Charlotte, NC offers a Nurse Home Visitor role within the Nurse Family Partnership (NFP) program. This 1.0 FTE, salaried exempt position reports to a Nurse Supervisor and carries a starting salary of $63,000. Bilingual Spanish and English preferred.

NFP focuses on supported home visits from pregnancy through a child’s second birthday, with nurses guiding families, providing education and referrals, and promoting positive outcomes.

Formación

• Covid-19 vaccination is required.
• Current unencumbered NC or SC RN license.
• BSN required.
• Current CPR required.
• Excellent written and verbal communication skills.
• Valid driver's license and insured automobile required.
• Must be able to work a flexible schedule including some evenings and weekends.
• Bilingual Spanish preferred—if hired as Spanish speaking candidate, fluent in Spanish.

Responsabilidades

• Provides home visits to women and their families eligible for the NFP Program with caseload of 25 clients.
• Adheres to nursing process and the NFP model of home visitation.
• Understands, supports, represents, and coaches others in the Nurse-Family Partnership vision, mission, and model in actions and verbally to both internal and external customers and colleagues.
• Assists in creating a positive work environment that promotes productivity, mentoring, teamwork, cooperation, and consideration of differing viewpoints.
• Develops therapeutic relationships, utilize concepts of reflection and motivational interviewing with women and their families in a home visiting environment.
• Assesses physical, emotional, social and environmental needs of women and their families as they relate to the NFP domains.
• Assists women and their families in establishing goals and outcomes.
• Provides education, support and referral resources in assisting women and their families in attaining their targeted goals.
